Reading Disorders & Psychiatric Co-Morbidity

10/16/2022

Conjecture
  • The Connections between:
    • Neuro-Developmental Differences such as ADHD, Autism and Dyslexia and Comorbidity of Psychiatric Disorders, and
    • Developmental Inflammation and Neuro-Developmental & Psychiatric Disorders are becoming fairly well established.

  • This has RAMIFICATIONS obviously for TREATMENT ---
    • BUT also for getting out of the MODERN MYTHOLOGY of the BRAIN ---

  •  Our populations have serious, widespread issues with DEVELOPMENTAL INFLAMMATION.

  • Early Diagnosis and Support are CRITICAL for people with Neuro-Developmental Differences associated with significant CO-MORBIDITY with Psychiatric Disorders.

  • There often are GIFTS --- BUT the CHALLENGES are there too ---- and it can be quite the TRAP FOR THE UNWARY.

"REASONABLENESS" as a Driver of "TOXICITY"

10/13/2022

Conjecture

Honestly, most people are pretty good at accommodating or working around the DIFFERENCES of others IF:
  • they know what those DIFFERENCES are and
  • they know how to accommodate or work around them.

  • The truth is many Neuro-Diverse people are NOT AWARE or AWARE ENOUGH of their own EXECUTIVE FUNCTIONING CHALLENGES ---

  • What we tend to use as a SHORT-CUT ---- is some general notion of "REASONABLENESS" for:
    • Short Term Memory
    • Planning
    • Organization
    • Energy
    • Emotional Regulation
    • Etc.

  • That can turn out to be PROBLEMATIC on the mild end or DISASTROUS on the far end--- because for Neuro-Diverse people they often have IDIOSYNCRATIC DIFFERENCES in EXECUTIVE FUNCTIONING.

  • A big part of the CHALLENGE of NEURO-DIVERSITY is that it is not just at one level of the society --- it's at the bottom, the middle and the top ---
    • and we tend to have a fair amount of UNCONSCIOUS BIAS regarding from whom we will TOLERATE EXECUTIVE FUNCTIONING CHALLENGES.

  • Some "RESILIENCE" researchers do make the point that what we're trying to do is get to an ACCURATE VIEW ---
    • That has been elusive in the old but newly appreciated realm of Neuro-Diversity

  • When it is not appreciated and we don't have an ACCURATE VIEW ---- "TOXICITY" either INDIVIDUALLY or in RELATIONSHIPS or in the SOCIETY I think can be a consequence.

ADHD & Exhaustion

One of the big insights of the last few years is that many people do not fit neatly into DSM categories and researchers are advising that there are shared genes and categories are BLURRED.

We don't want to be:
  • Demonizing people for EXECUTIVE FUNCTIONING CHALLENGES --
  • But we also don't want to be BLAMING others for our EXECUTIVE FUNCTIONING CHALLENGES.
  • AND we want as ACCURATE a view as we can get both of ourselves and of others
I often post videos from Asperger's From the Inside (now Autism From the Inside) -- and Paul makes the point that once people understand that his challenges are REAL --- the world is a much KINDER place.Additionally, in the words of Canadian Psychologist Tayyab Rashid --- "STRENGTHS ARE REAL."
Understanding REAL EXECUTIVE FUNCTIONING CHALLENGES and REAL STRENGTHS --- has the potential to ---
  • get us out of "TOXICITY"
  • and to a more accurate view of ourselves and others and our society
  • hopefully leading to more KINDNESS & RESILIENCE.

"Empathy" in ADHD and Autism

9/20/2022

Things are lot more complicated than they seem -- and there are Sex Differences
Conjecture
  • One of the ERRORS we have made is viewing EMOTION and LOGIC/RATIONALITY as OPPOSITES ---
    • Emotion is part of the Reasoning Process
    • There's enormous information in Emotion ---
    • BUT we don't have an UNLIMITED ABILITY to take that on.

  • What we've been calling "EMPATHY" appears to really be the brain's PRE-CONSCIOUS determination of whether someone is on our team.

  • Neurodivergent Therapist Louise Taylor's "EMPATHY of EFFORT" ----- we're just in the beginning stages of developing and it is a BIG COST --
    • BOTH Emotionally and
    • from a Reasoning standpoint AND
    • from an ENERGY standpoint.

[Is "DOUBLE EMPATHY" --- "LOVING YOUR ENEMY" --- it might be]

  • Neuro-Diversity is often characterized by:
    • RAMPED UP STRESS RESPONSES
    • RAMPED UP EMOTIONS
    • & at least sometimes RAMPED UP RATIONALITY to deal with all that.
Neuro-Diverse Therapist Louise Taylor on "EMPATHY"
If Autism and ADHD are under-diagnosed in girls and women, anxiety and classic "mental illness" are often under-diagnosed in boys and men ---

You can really appreciate the IRONY and RAMIFICATIONS of this when you realize NEURO-DEVELOPMENTAL DISORDERS such as ADHD and Autism are associated with PSYCHIATRIC DISORDERS.

What Do I Mean ---
  • We don't have any "UNLIMITED" ability as "INDIVIDUALS" to take on Sensory Information, Emotion --- and REASON our way through all that.

We're relying more than we have realized on UNCONSCIOUS BEHAVIOR  and JUDGMENTS:
  • BUT --- it's a SHORT-CUT ---- and like a lot of "SHORT-CUTS" it can miss things and get things "WRONG"
  • We consciously process 40 to 50 bits of information for every 11 MILLION we take in according to some estimates in 2022----
    • Even if you are processing a lot more information than the "AVERAGE" person --- and are aware of "MORE LOGICAL PROBLEMS and INCONSISTENCIES"
    • It is still a RELATIVELY SMALL/TINY AMOUNT OF INFORMATION ---
    • AND EVEN THAT MODEST INCREASE IN PROCESSING --- is coming AT A HUGE ENERGY COST.

  • INTEGRATIVE THINKING / RECOGNIZING INTEGRATIVE COMPLEXITY is NOT EASY for US and that's why we don't do more of it.

  • We've lost an HISTORICAL UNDERSTANDING of HUMAN EXISTENCE --- because we thought we were so ADVANCED we didn't need it.

  • WE NEED THAT LONGER VIEW --- because we're NOT AS FAR A LONG AS WE THINK --- and at 40 to 50 bits of information for every 11 Million (and that 11 Million doesn't represent all of reality)
    • WE GOT A LONG WAY TO GO.

  • FAITH/REASON/EMPIRICISM --- We're gonna need it all for the LONG ROAD AHEAD --- that as INDIVIDUALS we only travel a tiny portion of.

Neuro-Diversity and "Personality Disorders"

9/18/2022

Conjecture
  • For people with IQs over 70 and with ADHD, Autism, Dyslexia, Giftedness, OCD, etc. there are a lot of CHALLENGES to fitting into a NEURO-TYPICAL WORLD in which the ACCOMMODATIONS & WORK-AROUNDS you need are NOT ON THE TABLE.

  • Further, many people with some NEURO-DEVELOPMENTAL DIFFERENCE related to greater Neuro-Developmental Inflammation ---
    • Don't get that DIAGNOSIS until they are an ADULT ---
    • And many people never get that DIAGNOSIS.

  • We're more recognizing that the Neuro-Typical Human Brain doesn't completely "MATURE" until about 25.
For people with some form of Neuro-Diversity mentioned above --- their brains are probably not "MATURING" until 30.
  • a lot of Neuro-Diverse people with the RESOURCES go to:
    • Graduate School
    • Law School
    • Med School
  • Those ENVIRONMENTS provide STRUCTURE and can play to the STRENGTHS of NEURO-DIVERSE people
  • There are a lot of Neuro-Diverse people in ACADEMIA.
A very few ADULTS in our SOCIETY are getting SIGNIFICANT SUPPORT ---
  • and that is STRICTLY RATIONED.
  • It's largely based on MONEY & HIERARCHY.
  • Further, those ADULTS tend to be WEALTHY NEURO-DIVERSE People who are MEGA WEALTHY now -- but started out as wealthy
    •  a seeming Sweetheart like Richard Branson and a seeming nut case like Elon Musk.
When you look at the OTHER EXTREME --- Homelessness and Incarceration ---
  • One sometimes to often finds gifted musicians or people who want to develop their musical abilities.
  • What's an ABILITY IN MUSIC --
    • It's an ENHANCED ABILITY to RECOGNIZE PATTERNS -- MATH.
Picture
Thelonious Monk
Finally, the "PERSONALITY DISORDERS."

The CHALLENGE of NEURO-DIVERSITY is that it is often coming with BIG STRENGTHS and EXECUTIVE FUNCTIONING CHALLENGES.
  • If someone is high enough up on the PUBLIC HIERARCHY --- you can see this played out in Operatic Fashion --
    • Think Bill Clinton or Donald Trump.
I think "PERSONALITY DISORDERS" are sort of that FIRST LINE of OBSERVATION of a PROBLEMATIC BEHAVIOR ---
  • it's not necessarily "WRONG" as far it goes
  • BUT it doesn't go very far
  • and we've been AFRAID to address DEVELOPMENTAL ISSUES for fear of coming off as --
    • NAZIS
    • So I'm not saying there aren't complicated SOCIAL and ENVIRONMENTAL and DISCRIMINATORY FORCES at play --
    • But they are also playing out in a BIOLOGICAL CONTEXT of:
      • Executive Functioning Challenges.
      • Ramped Up Stress Responses and Sensory Processing
      • Substantive Strengths and
      • Epigenetics
A large MIDDLE CLASS helps to MODERATE the STRESS people are feeling ---
  • A lot of INCOME INEQUALITY and SOCIAL INEQUALITY --- ramps that STRESS UP
    • So HYPER-COMPETITIVE SOCIETIES might see some gains in some areas --
    • BUT I would submit they are also seeing some REGRESSION in other areas involving SOCIAL BEHAVIOR.
Based on the RESEARCH --- most of these people who are being labeled with a "personality disorder" are Neuro-Diverse
  • they are the ULTIMATE "MIXED BAG"
  • they need some SUPPORT in MANAGING this
  • and we all need to get much more SOPHISTICATED in our THINKING.
  • part of that is recognizing the UNCONSCIOUS BIAS that prevents us from recognizing COMMON PUBLIC HEALTH issues that are congregating at the EXTREMES.

Neuro-Diversity, Food Allergies & A Major Breakthrough*

9/15/2022

Conjecture

Food allergies can be a not so innocuous source of
INFLAMMATION.

Further, people with FOOD ALLERGIES are more subject to PSYCHIATRIC DISORDERS ---

AND

Neuro-Diverse people are more vulnerable to FOOD ALLERGIES & PSYCHIATRIC DISORDERS.

One of the BIG KEYS to this is ---
IT'S DEVELOPMENTAL

Another BIG KEY is that ---
IT'S A MIXED BAG

Third Big Key -- The MIXED BAG of NEURO-DIVERSITY is EXPOSING "UNCONSCIOUS BIAS" like perhaps nothing else could.

Some new research shows that allergies are linked to autism. Food allergies can make behavioral symptoms worse, in part because your child may feel uncomfortable and not know how to communicate this effectively.

Autism has also been linked to immune system problems, gastrointestinal issues, and changes to gut microbes, which may be due to sensitivity to certain foods or even food allergies.

Although many people with dietary allergies experience mild symptoms when exposed to triggering foods, some face potentially fatal consequences. A bacterial compound called butyrate that’s made by healthy microbiomes has shown promise against allergic reactions in lab tests, but it’s nasty to take orally.

Today, scientists describe a more palatable way to deliver this compound and report that their “polymeric micelles” are effective against peanut allergies in mice.

The treatment could someday counteract many types of food allergies and inflammatory diseases.

ASYNCHRONY and the Challenge of FLOW

9/5/2022

The Complicated Array of Physical, Social, Cognitive and Emotional Abilities and Challenges of the ASYNCHRONOUS Person  ---

Mean BIG STRENGTHS and BIG WEAKNESSES are the NORM --- NOT THE EXCEPTION.

Conjecture

It takes a lot of ENERGY to UNDERSTAND the IDIOSYNCRATIC ASYNCHRONOUS PERSON --
  • EVEN FOR THAT PERSON --- much less anyone else.

  • One of the reasons it is TAKING SO LONG is we tend to DEFAULT to the PERSON is either ACROSS THE BOARD BELOW AVERAGE or ACROSS THE BOARD ABOVE AVERAGE ---
    • Both are DANGEROUS DEFAULTS.

I would say that some of these BIG STRENGTHS are the result of greater DEVELOPMENTAL SYSTEMIC INFLAMMATION that is leading to SENSORY PROCESSING that is putting more DEMANDS on the person.

One of the places you see the DEMANDS is EXECUTIVE FUNCTIONING.
  • Was Executive Functioning damaged by excessive Developmental Inflammation?
  • Are the Executive Functioning challenges the result of greater damands of Sensory Processing?
  • Both? It's Individual?
  • I don't know -- BUT ---
    • Developmental Systemic Inflammation
    • Sensory Processing Differences
    • Executive Functioning Challenges and
    • Some BIG STRENGTHS
    • often go together.

  • There is a lot of SHARED COMMONALITY of INTEREST in getting ASYNCHRONOUS PEOPLE in their STRENGTHS.
    • It doesn't ELIMINATE EXECUTIVE FUNCTIONING CHALLENGES --
      • But it is generally going to REDUCE THEM.

  • "FLOW" is the state that most people are trying to reach that is a BALANCE of their individual STRENGTHS and WEAKNESSES.
    • You can see how hard this would be for the ASYNCHRONOUS PERSON and EVERYONE ELSE to do with the LIGHTS OFF or IN IGNORANCE.

  • Beyond some IDEALIZED STATE OF FLOW ---
    • It can be excruciatingly and dangerously difficult for the ASYNCHRONOUS person to stay within HOMEOSTASIS.

  •  This requires an INDIVIDUALIZED APPROACH for:
    • Families
    • Education and
    • Employment

  • CATEGORIES can be a helpful JUMPING OFF POINT --- but it's just the beginning.
    • Generally, we're talking about SHARED GENES across NEURO-DEVELOPMENTAL and PSYCHIATRIC CATEGORIES, and
    • IDIOSYNCRATIC MATERNAL IMMUNE ACTIVATION and AFTER-ACQUIRED INFLAMMATION.

  • So ONE-SIZE is not going to fit all in the FAMILY, EDUCATION or EMPLOYMENT.

Neuro-Diversity and the Demands on Individuals, Families & Societies to do SHORT, MEDIUM & LONG TERM PLANNING to Avoid Burnout & Breakdown
The Healthcare and Criminal Justice Costs of NOT UNDERSTANDING Neuro-Diversity are HUGE ---
  • BUT this turns out to be LEGITIMATELY VERY COMPLICATED.
  • When it comes to EMPLOYMENT/ENTREPRENEURSHIP --- Individuals, Families & Societies have a BIG INTEREST in figuring this out for the often ASYNCHRONOUS, IDIOSYNCRATIC NEURO-DIVERSE PERSON.

  • For me personally, I can't wait years or decades for others to keep up with my ABSTRACT THINKING   --

One of the things that you see often in job announcements is a requirement for "ATTENTION TO DETAIL."
  • That is very much a matter of NEURO-DIVERSITY.

If we get it in our heads, as we tend to do --- that this "ATTENTION TO DETAIL"  is a fundamental competency or even moral issue ---
  • We've erected some BIG BARRIERS for a SEGMENT of the NEURO-DIVERSE population.
  • The irony is that the most sanctimonious people on this "ATTENTION TO DETAIL" --
    • can also be quite NEURO-DIVERSE.

I think this issue of "ATTENTION TO DETAIL" that we think is so GENERIC  is actually quite INDIVIDUAL and IDIOSYNCRATIC.

For me personally, I'm interested in understanding enough of the details so I understand the underlying GENERAL PRINCIPLES.
  • After that, I'm looking for ANOMALIES that would go to the UNDERLYING GENERAL PRINCIPLES.
  • I'm pretty good at understanding a system and even working individually with clients to better understand where that system may be falling apart---- BUT --
  • That is generally a DIFFERENT KIND OF ATTENTION TO DETAIL than most Employers are looking for.

  • Further, some of our traditional "ATTENTION TO DETAIL" is just silly --- BUT SOME OF IT ISN'T.

If we understand --- and we generally don't understand --- that not only we ourselves but human beings in general  are only perceiving a small fraction of REALITY ---We start to appreciate the importance of DIFFERENT TYPES OF ATTENTION and SENSORY/INFORMATION PROCESSING.

Big Gifts, Big Energy Demands, Executive Functioning Challenges & Traps for the Unwary

8/15/2022

Conjecture

People with ADHD, Autism and/or Dyslexia are often coming with some BIG GIFTS and Executive Functioning Challenges that come with perhaps understandable but often destructive reactions from others or even ourselves:
  • "If you can do that --- I know you can do this." OR
  • "Your Executive Functioning isn't up to par -- You're An Idiot."
Those BIG GIFTS are often the result of some RAMPED UP SENSORY PROCESSING that is coming at a BIG ENERGY COST.
  • See Asperger's From the Inside
Why So Tired? How Sensory Processing Effects Executive Function

  • Further, REASONING comes at a BIG ENERGY COST.

  • Additionally, EMOTION is a part of the Reasoning Process.

  • The "FATIGUE" that comes with all this doesn't necessarily look like what we're expecting ---

  • Further, most of the people around the world are drinking a lot of CAFFEINE ---it can RAMP UP YOUR INTELLECTUAL FUNCTIONING ---
    • Unfortunately, it ramps up that STRESS RESPONSE, too.
There are some people like myself --- I have had a panic attack -- BUT if I cut out the CAFFEINE --- that goes away --- that is not true for everyone.
  • BUT that CAFFEINE helps my EXECUTIVE FUNCTIONING --- so that's the CATCH-22
  • Finding that BALANCE --- has been no easy trick.
  • BTW --- I do take Lithium
Picture
Some studies have found that caffeine can boost concentration for people with ADHD.   Since it's a stimulant drug, it mimics some of the effects of stronger stimulants used to treat ADHD, such as amphetamine medications. However, caffeine alone is less effective than prescription medications.

There's so much to this --- the Colorful happy background  --- but in the foreground is her ANXIETY on a cell phone --- she looks tired and her ANXIETY is handing her a cup of coffee.

As she is running from one thing to another --- her ANXIETY is trying NOT TO SPILL THE COFFEE.

For Neuro-Diverse people with EXECUTIVE FUNCTIONING DIFFERENCES in a SOCIETY that largely doesn't think it should have to ACCOMMODATE EXECUTIVE FUNCTIONING DIFFERENCES----

The Problematic Dynamics of Empathy, Stress & Ramped Up Sensory Processing

Research on Recognizing Facial Emotion Expressions Could Change Our Understanding of Autism

August 2, 2022

Summary:  Autism-associated social difficulties may reflect differences that only become apparent in high-pressure scenarios and certain social interactions. The findings challenge the belief that those with ASD can’t adequately read facial emotional expressions.

Source:  Flinders University

So I have never really bought this "IDEA" that people with ADHD &/or Autism couldn't read facial expressions---- I think it can be quite the OPPOSITE ---

BUT

There are PROBLEMATIC DYNAMICS --- I think it is more related to RAMPED UP STRESS RESPONSES that can get out of control.

Further, I'm fine if we want to FOCUS on EMPATHY but it's not going to get us very far if it is not the DOUBLE EMPATHY of EFFORT rather than the split-second EMPATHY of which team you're on.

I think it can be helpful to bring in this concept of REJECTION SENSITIVE DYSPHORIA which is often associated with ADHD, but I think cuts across a lot of NEURO-DIVERSITY that appears to be associated with greater Neuro-Developmental Inflammation.

Further, I don't think this issue of emotions normal in every way except INTENSITY is limited to "REJECTION."
  • Whether we call this "EMOTIONAL INTENSITY" associated with "Giftedness" or "EMOTIONAL DYSREGULATION" associated with a Neuro-Developmental Difference or Psychiatric Disorder ----
    • Can reflect a lot of "UNCONSCIOUS BIAS"

This INTENSITY is not all a bad thing, but it is not all a good thing either.
  • Further, I think it can become very hard on not only those around us but on our own bodily and mental systems.
Further, if we start to consider "PSYCHIATRIC DISORDERS" as related to Neuro-Developmental Differences/Disorders ---
  • there are more problematic dynamics.

ADHD, Autism, Bipolar Disorder, Depression and Schizophrenia are BLURRED, according to the US National Institute of Mental Health.

A particularly problematic behavior that has been associated with "BIPOLAR DISORDER" is ---
  • AGGRESSION as a means to stave off DEPRESSION
    • “Aggression protects against the onset of major depressive episodes in individuals with bipolar spectrum disorder.  Journal of Behavior Therapy (2017)

I think the problem is a lot less that people aren't sensitive --- BUT THAT THEY ARE TOO SENSITIVE.

I think that goes to some extent to explain the popularity of Mark Manson's books which I think are tapping into a counter-intuitive reality.

Picture
Flinders University --- Adelaide, Australia
Picture
Further, I think something that you hear Older Women say --- "Oh men are such babies -- they're so sensitive."

I don't think as girls and younger women we start out thinking that --- BUT I do think men are often more "SENSITIVE" and whether they are IMPLODING or EXPLODING --- it can be very problematic.

Especially, in the CRIMINAL JUSTICE SYSTEM where you often have men who are acting aggressively maybe as an unconscious way to stave off DEPRESSION---
  • AN OUNCE OF PREVENTION IS WORTH A POUND OF CURE
  • AND Both the PREVENTION and CURE may be COUNTER-INTUITIVE.
